Teachers receive a great many cards saying thank you for everything. The ones they keep are the ones that name a single moment they had probably forgotten, and tell them it mattered.
Three things that make it work
Name the moment
The comment in the margin. The time they kept you back and it turned out to be kindness. One incident beats a term's worth of gratitude.
Say what changed
What you can do now that you could not before. That is the whole of the job, and it is rarely said back to them.
Keep it usable
They may pin it up. Short, clean and free of anything that only makes sense to your class.
What to avoid
Shaping young minds and lighting fires. Teachers have heard both at every assembly of their careers.
An example
You wrote "say more about this" beside one paragraph in the second week of a year I had planned to waste. I have said more about this for eleven years. You will not remember. I am telling you anyway.
