Writing a apology poem
Most written apologies fail the same way: they spend more words on why it happened than on what happened. A poem is a useful form for this precisely because it is too short to hide an excuse in.
Three things that make it work
Say what you did, plainly
Name the thing without softening it. Any hedging reads as a defence, and a defence is not an apology.
Do not ask for anything
No requests for forgiveness, no hints about moving on. An apology that comes with a request attached is a negotiation.
Show you understand the effect
Not how bad you feel. What it was like for them. That is the part that proves you have actually thought about it.
What to avoid
"I'm sorry if you felt" and every other construction that hands the problem back to the person you hurt.
